Why this platform exists
ReferralPathway was founded by a healthcare professional with over 20 years of experience across the senior care continuum — spanning marketing, admissions, operations, and administration. That depth of experience means we've sat on every side of the referral process: as the social worker calling facilities at 4pm on a Friday, as the admissions director fielding those calls, and as the administrator watching placements fall through the cracks.
The problem was always the same: no shared system. Social workers maintained personal contact lists. Facilities had no reliable way to signal availability. Families were handed a printed sheet and told to start calling. Referrals were lost to voicemail, fax machines, and spreadsheets.
